Neftali Serrano, PsyD is our Chief Executive Officer, bringing over two decades of clinical, program development, and leadership experience to CFHA’s community. Dr. Serrano helps staff, membership, and our board of directors operationalize CFHAs vision to integrate physical and behavioral health across the US health system.
Dr. Serrano is perhaps best known for being a tech geek. He was an early adopter of using YouTube videos for educating professionals in PCBH in the mid-2000s and continues to annoy staff with new applications for technology. On a serious note, Dr. Serrano has served as Chief Behavioral Health Officer and founder of several integrated care programs at FQHCs throughout his 22+year career as a primary care psychologist.
Soccer or futbol, is Dr. Serrano’s weekend passion. As a center-midfielder for his adult soccer team and occasional coach for his son, soccer is a mainstay of the Serrano household.

Tawnya Louder, BA is our Director of Finance, bringing two+ decades of business management experience to CFHA. In this role, she leads the financial operations for the organization to include reporting, forecasting, budgeting, managing the day-to-day accounting & finances, and providing advice to inform financial decisions to sustain the vision of CFHA. She also manages the CFHA Community Conversations which seek to further community through member engagement around high-interest topics pertinent to Integrated Behavioral Health.
With a natural affinity for relationship building & business development, Tawnya has experience owning & managing service businesses as well as non-profit member association management and is a certified paralegal with specialties in contracts and real estate law.
A native South Dakotan who’s called NC home for 25+ years, Tawnya is an avid outdoor person who enjoys biking, hiking, paddling, running, & dragon boat racing. Her roots for family & community run deep; she can often be found supporting her boys in their activities or volunteering at school or within the community.
Jackie Poor Hahn, BA

Jackie Poor Hahn, BA is the Director of Events & Membership for CFHA. She manages most aspects of the conference including site selection, planning committee management, registration, speakers, abstracts, sponsorship, onsite management, and building the conference app. As the membership manager, she is the go-to person for any membership questions such as how to set up an organizational membership, renew membership, invoicing, etc. She is at most public meetings as the SIG and Committee liaison.
Jackie has almost 30 years of experience working with associations, having held a variety of roles through the years, but mostly in conference management. She spent 18 years working with an organization to promote affordable housing in Colorado. About 15 years ago she founded a charter school for kids who were at risk in traditional school environments, providing a gifted education to all students. She was Board President for the school and helped oversee the purchase of a permanent home as well as building out the school from kindergarten through high school.
She graduated from SUNY Stony Brook many moons ago and worked as a legal assistant in New York managing large product liability cases at the beginning of her career.
Jackie enjoys traveling and trying new restaurants with her husband of 30 years. Originally from New York, she now lives in Colorado but loves to go back to New York to visit. Although she lives in a beautiful state with lots of hiking and great things to do, she is a city person and would much prefer marching through a big city to hiking a mountain. She has three grown sons that make her laugh, bring her pride and joy, and she enjoys developing a new adult relationship with them. Much of her free time is spent researching the next place she wants to travel to.
Martha Saucedo, LCSW

Martha Saucedo, LCSW, brings over 14 years of profound expertise in the integrated care field, where she oversees operations and initiatives at CFHA’s Technical Assistance branch. Her fervent dedication to knowledge-sharing serves as the driving force behind her instrumental role in conceiving and executing initiatives that enhance the caliber and impact of Integrated Health services on a national stage. Additionally, Martha meticulously curates CFHA’s content across various platforms, including events, live and virtual conferences, ensuring it not only enriches but also surpasses members’ expectations with invaluable and captivating material.
After earning a degree in counseling psychology in Monterrey, Mexico, Martha relocated to the United States in 2000 due to her husband’s IT job. This move necessitated a restart of her career in the U.S. healthcare system, an industry she had always been drawn to. Martha subsequently obtained her Licensed Clinical Social Work degree and began her journey in Integrated Care at an FQHC in Madison, Wisconsin. There, she continues to serve as a dedicated Behavioral Health Consultant and also holds the role of the Latinx Community Liaison. With a deep sense of pride in her Latina heritage, Martha remains resolutely committed to promoting diversity, inclusion, and empowering minority groups, not only in the community, but also within the workforce.
Martha, a devoted mother of two, finds joy in her family’s diverse passions. Her son, a dedicated baseball enthusiast, nurtures young talent through his Baseball Academy. Meanwhile, her daughter expresses herself eloquently through dance. Martha and her husband relish biking and nature walks together. She finds solace in spiritual texts and cherishes daily meditation sessions with her loyal Tuxedo cat, grounding her amidst life’s whirlwind.

Daniela Vela Hernandez, LMFT is part of our Technical Assistance team as a Technical Assistance Associate and supports clinics with behavioral health integration implementation and workflow optimization.
Prior to joining the CFHA team, Daniela developed a strong clinical background and experience in multiple settings from community mental health clinics to startup behavioral health. She is passionate about making mental health services accessible to all and is able to use her knowledge of traditional mental health services and integrated behavioral health to support the growth and implementation of integrated care.
Daniela grew up in San Diego, but now resides in little-known Imperial, CA with her husband, two children, and two pups. When not supporting the CFHA mission, Daniela is out rooting for her kiddos on the field.

Family Medicine Physician Jennifer Thomas, MD and Psychiatrist Laura Sidari, MD dually serve in the role of CFHA’s National Medical Director for Integrated Care. Dr. Sidari and Dr. Thomas bring to CFHA more than 20 years of combined clinical and leadership experience in integrated healthcare. Together, they lead CFHA initiatives that meaningfully bring together medical professionals across diverse integrated care settings. They contribute invaluable insights and expertise to multiple CFHA teaching, technical assistance, and advocacy projects that stretch nationally.
Laura Sidari, MD is a board-certified general psychiatrist with additional specialized training and experience in integrated behavioral healthcare. She has leadership and clinical experience in integrated care development and delivery across military, college health, and community care settings. In addition to her role for CFHA as Co-Medical Director, she is clinically active as a Consulting Psychiatrist and Director for Integrated Behavioral Healthcare services for regional not-for-profit health system in Central NY. Dr. Sidari also serves as Co-Chair for CFHA’s Collaborative Care Model Special Interest Group. Dr. Sidari is passionate about helping healthcare teams and professionals to be effective in practical applications of the evidence-base for integrated care, and she has national experience as an integrated care technical consultant.
Dr. Sidari lives in beautiful Ithaca, NY with her 3 living children, husband, and Cavalier King Charles Spaniel. Her eldest son, Leon, died at age 4 due to complications from influenza, and she speaks openly about her journey into integrated care beginning at her life’s intersection with child loss. She enjoys engaging with others in creative initiatives related to photography, writing, and traveling that enrich and inspire her work in integrated care.

Jennifer Thomas, MD, FASAM brings over 14 years of experience in primary care to the CFHA team. As a board-certified physician in family medicine and addiction medicine. Dr. Thomas practices in a community-based, rural setting in northern Illinois and serves as the founder and Medical Director of Integrated Behavioral Health at Morris Hospital. As a leader in developing partnerships between academic research settings and community-based providers, Dr. Thomas works as an Affiliate Trainer for the University of Washington AIMS Center to support fellow clinicians in developing Integrated Care in their practice settings. In addition to primary care and behavioral health, Dr. Thomas is passionate about incorporating treatment for Substance Use Disorders into the primary care setting.
Dr. Thomas currently resides in Illinois with her husband and four children. When she’s not working on Integrated Care initiatives, Dr. Thomas loves baking pies and working in her flower garden.
Leiana Edwards, BS

Leiana Edwards, BS is our Lead for Logistics & Design. She started working for CFHA in 2018 as an administrative assistant, but over the years, her role became more focused on communications and design. Now, Leiana works to oversee the logistics and design processes for the organization. This includes but is not limited to web development and management, creating marketing plans, maintaining the organization’s online platforms, engaging with the organization’s members to start new initiatives, and maintaining a consistent organizational brand identity.
Leiana graduated from the University of North Carolina with a BS in Health Policy and Management from the Gillings School of Global Public Health. Her passion for public health and clinical care, combined with her love for design makes her very excited about her work at CFHA.
Leiana has relocated to New York City in 2022 from North Carolina, and when she is not working, she enjoys spending her time getting to know her new city. In her free time, Leiana enjoys reading, cooking, puzzles, hand-lettering and designing personal work, the occasional home decor or DIY project, or going for long walks in Central Park.
Hannah Morse, BA

Hannah Morse, BA is our Administrative Coordinator. She is in charge of administrative logistics such as scheduling meetings via Zoom, taking minutes, editing meeting recordings, and updating calendars. She’s also responsible for coordinating with members, updating the membership database, and ensuring member access to the listserv.
Hannah graduated from the University of Colorado with a degree in International Affairs. Prior to CFHA, she worked as an Admissions Counselor at a medical and dental school. In addition, Hannah has experience in a variety of administrative roles, including database management.
Hannah is originally from the Denver area, although she has spent the past four years moving across the country with her partner as he completes medical school. This includes a two-year stint in rural Missouri, where she adopted her two cats. In 2021 she moved to the Valley of the Sun – Phoenix, Arizona – and has enjoyed being close to the mountains once again. In her free time, she likes to hike, play trivia, attend barre classes, and explore new restaurants.
