At CFHA, we are committed to community engagement and helping members bring their ideas to life. We provide a variety of options to support member-driven interests. Whether you’re looking to discuss a specific topic with colleagues in a one-time conversation or start a focused group on a particular subject in integrated care, there’s a way for your ideas to take shape at CFHA.
Community Conversations
Community Conversations offer integrated care colleagues a relaxed space to discuss high-interest topics, share experiences, and ask questions. As an entry-level and informal way to engage, these meetings encourage meaningful exchanges on a wide range of subjects. Not sure about leading the conversation yourself? Our Community Conversation Facilitator, Stacy Agosto, is available to help frame and guide discussions. We also frequently invite “Super Participants” — subject matter experts — to enrich these conversations and make it easy for members to get involved.
Have an idea for a Community Conversation topic? Email Stacy Agosto at stacyagostolcsw@gmail.com to learn more and get a conversation scheduled! You can also check out our calendar for future Community Conversations here or view old recordings here.
Targeted Trainings
Our targeted trainings or webinars offer a focused, webinar-style learning experience designed to deliver educational content on essential integrated care topics. These sessions feature knowledgeable presenters who guide participants through current best practices, new research findings, and practical strategies that members can apply to their integrated care work. Each training is crafted to be interactive, allowing time for Q&A so that attendees can deepen their understanding and gain real-world insights.
Targeted trainings are an excellent way to promote your expertise in specific areas, keeping you at the forefront of integrated care. Email us at info@cfha.net if you have an idea for a targeted training!
Workgroups
Our workgroups are mission-driven teams dedicated to advancing specific initiatives within integrated care. Each workgroup is formed around a focused goal, with clear endpoints or deliverables expected as the group’s outcome. These groups require committed leads and consistent participation from members over a set period to ensure progress and impact. Unlike our ongoing groups, workgroups are time-limited, disbanding once their objectives are met.
Starting or joining a workgroup is an excellent way to contribute your skills and make a measurable difference in a targeted area, all while collaborating closely with like-minded professionals.
View our workgroups here. If you are interested in starting or joining a workgroup, email us at info@cfha.net.
Special Interest Groups
Our Special Interest Groups (SIGs) bring together members around shared topics within integrated care, creating a space for long-term community building, collaboration, and knowledge sharing. Each SIG has a sustained focus, with regular meetings, discussions, and the development of valuable work products that benefit the broader community. SIGs are led by dedicated co-chairs and supported by additional volunteer roles, which rotate regularly to keep ideas fresh and inclusive.
Starting or joining a SIG allows you to connect deeply with peers who share your interests and contribute to the ongoing advancement of integrated care practices.
To start a SIG: Email us at info@cfha.net if you have an idea about a new SIG.
To join current SIG: If you want to get involved, attend the next SIG meeting of your choice. View all events here.
Content Creators Workgroup
The CFHA Content Creators Workgroup is dedicated to increasing the visibility and impact of integrated care through diverse media, including social media, blogs, podcasts, and videos. This group serves as a creative outlet for members to share their unique perspectives, expertise, and innovations in integrated care.
Whether you’re brainstorming individually or collaborating with others, you can develop ideas, gather peer feedback, and submit your content for review. Approved pieces are shared across CFHA’s platforms, reaching a broad audience and contributing to a growing library of member-driven content. For example, you can view our short form content here.

Conference Ideas
There are a number of ways to share your ideas, research, best practices at our annual conference:
- Concurrent Sessions: These structured, in-depth presentations provide an opportunity to showcase innovative research and best practices. Attendees can engage with presenters through interactive discussions. Our Call for Proposals is always February 1 – March 15th each year.
- Lunchtime Discussion Groups: These informal gatherings create a relaxed setting to explore high-interest topics, share experiences, and exchange ideas with colleagues over lunch. Participants can choose to lead a discussion or join an existing conversation.
- Extra Sessions: In the past, we’ve offered unique sessions like our Poetry and Prose event, adding a creative and reflective dimension to the conference. If you have an idea for a new conference add-on, we’d love to hear it!
